Services

We provide the following services to communities in Atlantic County, Southern Ocean County and Cape May County:

  • Physical, Emotional and spiritual support
  • Assistance with personal care as needed
  • Medication for symptom management
  • Med. equipment for care at home
  • Short term respite care
  • RN available by phone 24 hours a day
  • Bereavement follow-up

AtlantiCare Hospice Team

The AtlantiCare Hospice team is multidisciplinary. Whatever the medical need, we supply knowledgeable staff ready to provide care in the comfort of the patient's home. A patient's team consists of the following AtlantiCare staff:

  • Medical Director
  • RN Case Managers
  • Medical Social Workers
  • Certified Home Health Aides
  • Chaplain
  • Volunteers
  • Pharmacist

The patient's family or support group is also an essential part of hospice, typically they are the primary caregivers on a daily basis.
