(Newark, NJ – October 9, 2012) -- Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ) and AtlantiCare announced an agreement today to deliver more effective care at lower costs to over 1,500 Horizon BCBSNJ Medicare Advantage members in Southeastern New Jersey. The new Accountable Care Organization (ACO) agreement will initially include 25 primary care practice locations and 45 primary care doctors.
AtlantiCare and affiliated primary care practices will take on additional accountability for improving the health and patient satisfaction for Horizon BCBSNJ Medicare Advantage members. Horizon BCBSNJ will make incentive payments to AtlantiCare to improve patient care coordination. Participating practices that meet health improvement, cost, and patient satisfaction goals will have an opportunity to receive, as additional compensation, a share of the resulting savings of the ACO.

Practices within this ACO will be operating as Patient-Centered Medical Homes (PCMHs). Medical Homes take a coordinated and more patient-centered approach to delivering health care. Patients will receive personalized and comprehensive care through a lead physician, supported by care teams who are accountable for closely monitoring patients’ progress, engaging patients in their own health care and centrally coordinating the delivery of a patient’s care with other providers.
This initiative is open to all current and future Horizon BCBSNJ Medicare Advantage members who select a doctor affiliated with this ACO. Participating ACO primary care practices are listed below.
