Completing Know Your Numbers has never been easier with three easy options to certify your weight and blood pressure.
- Visit an onsite kiosk located at Mainland- Wellness Room, City- Wellness Room, or Airport Commerce Center- Lighthouse Conference room.
- Complete your numbers with your provider at any visit using the Wellness Certification Form
- Join us for an Onsite Know Your Number Event.

Take advantage of a convenient, on-site opportunity to complete Know Your Numbers and earn Wellness Credits. The Wellness Team will be available to assist you with your numbers, provide helpful tips for staying aligned with your wellness goals, and share information about resources to support your well-being. No appointment is needed—just drop in!

Your health plan is committed to helping you achieve optimal health. Rewards for participating in this wellness program are available to all benefit eligible employees by way of wellness credits. If you think you might be unable to meet a standard for wellness credits under this wellness program, you might qualify for an opportunity to earn the same wellness credit by different means.
