Program of All-Inclusive Care for the Elderly (PACE) in New Jersey

As we age, we may find that certain activities are more difficult or limiting than they used to be – it's just part of life. But not everyone is ready to leave the home they love, the community they know and the independence they've had their whole life.

AtlantiCare LIFE Connection is a hybrid care model for people who are eligible for a nursing facility but wish to remain in their own home. As a Program of All-Inclusive Care for the Elderly (PACE) in New Jersey, AtlantiCare LIFE Connection is a nursing home alternative that gives you a personalized team of health care professionals to provide services to you in a setting of your choosing. We come to you to help with care coordination, transportation, social services, meals and any other day-to-day things you may need assistance with — but none of the ones you don't. Our goal is to keep you healthy and independent.
